Individuals
Purpose
A Community Health Worker (CHW) can direct the individual to many resources and help support them in their return to the community. Also, the Judge and attorneys greatly favor those who are in program.
Join our program free of charge for Medi-Cal members.
Join group online and access progress reports and a certificate of completion to give to the Judge, attorney or a future employer.
Health
Many of us neglect our health. The purpose of the group is to encourage each other and talk about what we go through each week. We will be checking in on your general health and discussing options for care.
Housing
Medi-Cal in California has a program called Cal-Aim, and the funds are used to navigate to stable housing. We can navigate this process and share insight. The counselor can advocate you and work with other organizations.
Food
Food is a big topic right now. There are actually several food banks, yet only one site is very well know. In group we will be sharing locations and times to pick up food. We will be scheduling the upcoming week together.
