for Members of
Populations with Disadvantaged Health Outcomes
Purpose
A Community Health Worker (CHW) can direct the individual to many resources and help support them in their journey to a positive health outcome and to a productive life.
Join our program free of charge for Medi-Cal members.
We support formerly incarcerated individuals, the LGBTQ community, former foster youth, homeless individuals, those with serious mental illness and/or serious medical disabilities or those with asthma.
Health
Many of us neglect our health. After an individual assessment, the purpose of the groups is to encourage each other and talk about what we go through each week. We will be checking in on your general health and discussing options for care.
Housing
Medi-Cal in California has a program called Cal-Aim, and the funds are used to navigate to stable housing. We can navigate this process and share insight. The counselor can advocate for you and work with other organizations to get your housing funded or stabolized.
Food
Food is a big topic right now. There are actually several food banks, yet only one site is very well know. In group we will be sharing locations and times to pick up food. We will be scheduling the upcoming week together.
